She's linked to Old Misery in the Graham Greene story, which Donnie references earlier in the film. When Donnie arrives he discovers it's his nemesis, the bully waiting at the Cellar Door. He must face up to the bully, this dark power in his subconscious. He must also face up to the fact that it is he who inadvertently kills Frank.

It is when he reaches the deepest level of his subconscious, where he's repressed the shade. This allows there to be a doorway for a dark entity to get in and wreak havoc on his life.
It is Roberta Sparrow's house that he's hidden away. He's hidden away the fact that he killed Frank, his sister's boyfriend, and he did it in a fit of rage for accidently running over Gretchen.
The Cellar door is the doorway to the basement in Jungian analysis. This is where we find the darkest most bestial aspects of our persona. Donnie must face his fears, his evil, by triumph of will.
